Transporting Clinical Data Is Not the Same as Transporting a Trustworthy Clinical Document
Structured data exchange moves computable facts. A trustworthy document also needs identity, presentation, provenance, lifecycle state and verification.
The bounded UD model
UDR records the changing document and lineage. UDS preserves an exact sealed state. BrowserCarrier carries a readable offline presentation and the original UD. Local document intelligence can answer from contained text with citations and refuse unsupported questions.
What this does not establish
Technical provenance and cryptographic integrity do not establish clinical truth, identity assurance, HIPAA compliance, medical necessity, patient-safety outcomes or regulatory approval. Production healthcare use requires a governed implementation around the document.
