Why Does a Clinical Document Lose Intelligence When It Leaves the EHR?
The originating system can hold workflow, authorship and revision context that a PDF export does not automatically carry. UD proposes a portable document layer for that context.
The bounded UD model
UDR records the changing document and lineage. UDS preserves an exact sealed state. BrowserCarrier carries a readable offline presentation and the original UD. Local document intelligence can answer from contained text with citations and refuse unsupported questions.
What this does not establish
Technical provenance and cryptographic integrity do not establish clinical truth, identity assurance, HIPAA compliance, medical necessity, patient-safety outcomes or regulatory approval. Production healthcare use requires a governed implementation around the document.
