A fictional care team creates a living UDR from structured headings, medication information, and follow-up instructions. Each accepted change becomes a new revision instead of silently replacing the prior record.
A compatible Reader can present the document for different audiences while keeping the underlying document portable. Any translation, identity, or clinical workflow assurance must be evaluated separately and is not implied by the file format.
When a final version is selected, a UDS can carry independently checkable integrity evidence. Signature validity, verified identity, trusted time, current status, and transparency remain separate trust states.
